Work Location and Work Description - This information was required before the permit could be saved. It can be edited as necessary but can't be cleared as the information is still required.

Planned Work Start (Required) - An initial proposed start date is now required before the permit can be requested.

Planned Work End (Required) - An initial proposed end date is now required before the permit can be requested.

Equipment - The assets that are going to be worked on.

Work Equipment and Tools - The equipment and tools that will be used for the work.

Supporting documents: Isolation Certificate, Confined Space Entry, and/or General Certificate - Indicate what supporting documents are going to be required for this work to take place.

Attachments - Attach all other supporting files and images as necessary.
Hot work permits have the following additional fields:

Gas Test Required - Yes or No

Every X hours - How frequently should the gas test be completed?

Continuous Gas Detection - Should there be continuous gas detection?

Risk Assessment (Guard Condition) - Refer to Risk assessment for details.

Select Submit Request - When the required fields and Risk Assessment are complete, and you want to submit the permit for approval.
