Escalated risk assessment

  1. Select the Escalated level.

  2. Select the person that is going to take LEAD responsibility for the risk assessment. Only the LEAD can assign their team members and edit the Risk Assessment section.

    As assigned lead:

  3. Select hazards and controls

  4. Select PPE items

  5. To calculate the risk score for each hazard, select Initial risk. The initial risk assessment window opens.

  6. Select the square in the RISK ASSESSMENT MATRIX that best describes the initial risk.

  7. Select CONFIRM.

  8. Select Residual risk. The residual risk assessment window opens.

    By default, each hazard has a residual risk score. However, the form can be configured to have a residual risk score for each control.

  9. Select the square in the RISK ASSESSMENT MATRIX that best describes the residual risk.

  10. Select CONFIRM.

    The initial and residual risks need to be specified for each selected hazard. The risk score is calculated and displayed (next to the ALARP declaration) after all the risks have been assessed.

  11. When all the relevant hazards, control, and PPE items have been selected and the risks are specified, select the ALARP (or your local equivalent) declaration checkbox.
