To create a new service request, the registered user clicks the Create Request button on the Request List screen.

While guest users are directed to the Service Request entry screen upon login where they can create a new request. In the process of creating a new service request, a guest user can also be registered as an EAM user for future access to the Service Request Portal.

The Create Request screen includes the following sections and corresponding fields:

Location Details section

Campus

The general location of the incident or issue.

Building

The building where the incident occurred or the issue is located.

Floor

The specific floor within the building.

Room/Unit

The specific room or location on the floor of the incident or issue.

The values for campus, building, and floor and room/unit are defaulted from the contact information record of the logged-on user.

A new organization option SREQDISP (Service Request Equipment Display) has been created to support the Service Request Portal. The value options for this parameter are ALL, or ASSOCIATED. With this setting, registered users can select the values for campus, building, floor and room/unit from all equipment values, or users can be limited to those values set on their contact information record.

Request Details section

Field

Description

Request Date

The date the service request is/was entered (system date).

Service Category

The service category for the request. Service categories are included in the drop-down if the Service Request Portal check box is selected on the Service Category screen.

Service Code

The service code for the request. Service codes are included in the drop-down if the Service Request Portal check box is selected on the Service Code screen.

Attached Documents

This check box indicates documents have been attached to the service request.

Request Details

Any additional information to describe the incident or issue as observed by the user.

Contact Details section

Fields/Button

Description

Name (First)

The first name of the user. For registered users, this value defaults to the name defined on the user contact information record.

Name (Last)

The last name of the user. For registered users, this value defaults to the name defined on the user contact information record.

Email

The email address of the user. For registered users, this value defaults to the email address defined on the user contact information record.

Phone

The phone number of the user. For registered users, this value defaults to the phone number defined on the user contact information record.

Request Registration

This check box allows guest users to request registration.

  • For registered users, the contact details are defaulted from the user’s contact information record and the Request Registration check box is hidden.

  • For guest users, the contact details default from the guest user contact information record. The install parameter SRDFLTID is used to identify the guest user contact record for the Service Request Portal.

  • For guest users, if Request Registration check box is selected, the service request is created with the new contact ID provided.

Upload Documents

Attaches documents to the service request. If the user clicks the Upload Documents button in insert mode, the service request record is saved first, and then in update mode, the Documents pop-up window displays a grid of documents attached to the service request.

Submit

Submits the service request for processing. Registered users are navigated to the Request List screen. Guest users are logged out of the Service Request Portal upon submitting a service request.

Submit and Add Another

Submits the current service request for processing and remains on the Create Request entry screen to enter another service request. If the user is a guest and did not register with their initial submission, the values populated in the contact details section are retained in the system.
