This report can be used to print the permit to work.

Selection Criteria:

  1. Required Start Date - Selects permit to work records where the Date Required is greater than or equal to the date entered for this selection parameter.

  2. Required End Date - Selects permit to work records where the Date Required is less than or equal to the date entered for this selection parameter.

  3. Include LOTO Tags - Will print tags. The system will print tags using color based on the selected Risk.

  4. Include Hazard Rating System - This will print a section with the details for the Hazard Rating System. The legend for the Health Hazard, Flammability, Instability, and Special Hazards codes that can be attached to Part Safety records. Source is the National Fire Protection Association.

Report Details

The report is organized in sections. There is a section for the permit to work details, safety details, user defined fields, comments, and LOTO. The LOTO section is split into the Isolation Steps and De-Isolation Steps.

LOTO, if selected, are printed in the quantity specified on the LOTO records. If the quantity is blank the report will not print tags for that LOTO record. The report will also only print a maximum of 50 tags, even if a greater quantity is entered.

Below are important details to note:

  • The printing of LOTO Tags can be controlled by the user. The fields Tag Header and Tag Line 1 through 4 of the LOTO record attached to the permit to work are printed on the tags to support personalization.

    • Tag Header is the tag name. If Tag Header is left blank the report will substitute the word, Danger.

    • Tag Line 1 through 4 are printed on the tag in sequence from top (Tag Line 1) to bottom (Tag Line 4). The font of the earlier lines is greater than that of lines 3 and 4.

    • Since the font size differs per line the number of characters than can be printed varies. Two lines are reserved per field. This means 25 characters for the Tag Header, 60 for Tag Line 1, 80 for Tag Line 2 and 150 for Tag Line 3 and 4. Below are some example screen shots (Red Risk). Actual tags may look different. Depending on the text the number of characters mentioned above may not be accurate. If more than two lines are needed, and therefore the tag does not fit the page anymore, try removing some of the characters.

  • You can also enter tag text on the Isolation Method. If the text for all the tags is mostly the same this would be easier to maintain. You can define the text for each individual Isolation Method, or you can define a default text per Organization.

  • The report will select the tag text as follows:

    If Tag Header or Tag Line 1 through 4 are entered on the LOTO record of the permit to work screen this text will be printed.

    • If all these fields are blank, it will print the values from the Isolation Method attached to the LOTO record if one or more tag fields are entered.

    • If still none found the Isolation Method flagged as the Default Tag of the permit to work Organization will be selected and used if one or more tag fields are entered.

    • If still none found the Isolation Method flagged as the Default Tag of the default Organization will be selected and used if one or more tag fields are entered.
